Applying for an Examination

The relevant Guidance Notes and the Structured Reference Form are below.

  1. Please have the following to hand when applying:
    • Debit/Credit Card
    • Three completed Structured Reference forms (Word or PDF format)
    • Curriculum Vitae (Word or PDF format)
    • Summary of Operative Experience (Word or PDF format)
    • Photocopy of Photographic Identification (Word or PDF format)

    The application process cannot be completed until all relevant documents have been uploaded.
